JOB DESCRIPTION
JOB TITLE: Support Worker
REPORTING TO: Social Worker, Project Co-ordinator and Director
Rate: £6.50-£7.00 per hour
Hours required: Sunday 20.00-09.00
Position is subject to satisfactory Enhanced CRB Disclosure and 5 year referencable employment history.
PURPOSE OF POST
A support worker is responsible for proving support to up to five Service Users who are under the care of social services, in one residential unit.
DUTIES AND RESPONSIBILITIES
This is not a comprehensive list of all tasks, which may be required of the post holder; it is illustrative of the general nature and level of responsibilities of the work to be undertaken
1) Provide support at the request of Service Users with regards to attending appointments and meetings in accordance with statutory regulations
2) Assist Service Users in their development of independence skills and document this in their life skills book
3) Participate with all professionals involved with the Service User in relation to their semi-independence needs/issues
4) Support the Service User in the applications for housing, employment, education and training
5) Support the Service User in obtaining relevant identification E.G.: Passport, Birth certificate, National Insurance Number, Provisional Driving Licence and Bank Account
6) Assist the Service User in maintaining their health by supporting them in the registration process of a GP, Dentist and Optician and keeping a record of appointments if requested by the Service User
7) Manage a workload effectively
8) Ensure appropriate help and guidance is given to the Service Users, objectively and appropriately, within a supportive relationship in keeping with legislation, company policies, guidance documents and procedures
9) Participate and contribute in meetings, training courses, staff development programs and supervision and appraisal sessions arranged by the project coordinator/Director
10) To be aware of and comply with all Company policies and procedures
11) Maintain and update your knowledge of legislation, Company policies, procedures, guidelines and other relevant information and ensure that these are adhered to
12) Participate in ensuring all health and safety requirements are met, in particularly:
• following instructions
• not causing harm to others through action or omission
• not misusing or tampering with equipment provided for safety
• reporting things you perceive to be dangerous
13) Report all health and safety issues to the project coordinator and where possible the remove or resolve the health and safety issue safely
14) Ensure the security of the building
15) Ensure that anti-social behaviour both in and outside the Unit is not conducted
16) Participate in the cleaning and maintenance of the Unit
17) Record who enters and leaves the unit
18) Ensure that all visitors have left the building by 2200 hrs
19) Report to the relevant agencies the failure of individual Service Users to return
20) Report the return of a Service User who has been reported as missing
